Declaration of Parent/Guardian for Minor Passports - Annexure C
When a single parent is applying for a passport for his/her child without getting the consent of his/her spouse, then Annexure C must be enclosed along with the other documents to process the passport application for the minor.
Annexure C is required when:
- You are a single parent (father or mother) wishing to apply for a minor passport.
- The minor’s other parent does not give consent to apply or sign for the passport.
Declaration of Parent/Guardian for Minor Passports - Annexure D
Annexure D is an affidavit enclosed by parents or legal guardians when applying for a minor passport. By this affidavit, they confirm that they take sole responsibility for the minor’s expenditure and that all information provided is true. Based on this, the authority decides and issues the passport for the minor.
